The Study of Critical Indicator Development for Establishing Patient Classification System in the NICU

Abstract
Purpose:To analyze critical indicators for the establishment of patient classification.

Method:The study involved following four steps. The first step listed nursing activities in the NICU. The second step measured the time of nursing activities from preparation, performance, and arrangement by continuous observation and the frequency of the each nursing activity for three shifts over randomizing ten days given to the each patient by the self-recording of the nurses. The third step was to classify the patients according to the daily nursing care time. The fourth step was the selection of the critical indicator for different patient classes.

Result:The nursing activities were classified to ten categories which was further divided 122 activities according to the situation The results of the length of time were nutrition care 28.45%, Observation & measurement 20.45%. The frequency of the nursing activities was Observation & measurement 44.56%, medication 10.42%. Patient classification according to the daily nursing time was decided by using average and standard deviation. Patients were classified to 4 classes. The critical indicators were selected by based on each class nursing activities percentile and frequency.

Conclusion:The critical indicators for the Class ¥° are T.P.R 3/day, I/O check, care of during NPO and central-line management, for the Class ¥± are oral medication, T.P.R 3/day, bottle feeding and discharge arrangement, for the Class ¥² are oxygen therapy, G-tube feeding, diaper weight check, incubator care, new-patient care and T.P.R 4/day, for the Class ¥³ are artificial respiration care, endotracheal suction, central-line management, A-line management, BP check, care of during NPO and TPN therapy.
